The undersigned USER (“You” and INDIVIDUAL/APPLICANT below) confirms that the information provided is true and complete and authorizes Guardian Leasing Corp to use this information in order to confirm your identity and evaluate your credit worthiness in relation to the application being entered into. If this vendor application is approved by GLC, the vendor understands and agrees that they may not be approved by all GLC underwriting affiliates, the undersigned authorizes GLC to collect, hold, use, exchange and disclose your personal information as required, in order to administer its account and any applicable vendor program agreement.
